Required Care Plans: Please complete and return a care plan appropriate for your student's medical concern. These can be found on our "Forms" page and encompass asthma, food allergies, other severe allergies (bee stings, medications, latex, etc.), diabetes, and seizures. The General Health Care Plan may be used for anything unmentioned (injuries, disabilities, cardiac conditions, etc.). Contact the health room with changes or concerns throughout the year.

Medical Concerns: Call the health room to report serious injuries, illnesses, or impending surgeries that may affect your student's performance in school. Call the guidance office to set up any required accomodations prior to his or her return.

Parent/Guardian Out of Town: Contact the front office stating how long you will be gone, where your student is staying, how we may contact you, and who is to act on your behalf while you are gone.
